Patient Request of Medical Records
Selfii is an innovative healthcare service that empowers patients to securely collect, manage, and use their personal healthcare information from multiple sources, including healthcare networks, providers, even from snapping a picture and sending it to Selfii. By centralizing your healthcare records in one place, Selfii enables you to take control of your healthcare journey, and use your data to receive AI-driven insights, and find relevant, innovative healthcare solutions for better healthcare management.
Selfii supports the Cures Gateway, which is a cloud-based service that enables patients to request their Clinical Health Data from Health Information Exchanges (HIEs) and connected provider networks. Selfii utilizes the Cures Gateway to securely search for and retrieve patient data from nationwide networks.
LANES provides two ways for you to begin the process of requesting your records: a QR code and a URL. LANES recommends that you use the provided QR code, which is supported by mobile devices. You will need to provide accepted documentation for identity verification (ID Verification) for which you will need to use your mobile device.

ID Verification
ID Verification is a process where the identity of an individual is verified. ID Verification can happen in-person or remotely. Selfii uses remote ID verification over the internet.
Each patient (or user) will be asked to upload a picture of a form of government issued identification (see “What forms of ID will I need for ID verification?) to Selfii. You will be then asked to pose for a selfie to “bind” the taken photo to the issued identification. Selfii uses CLEAR (https://www.clearme.com) to verify your identity with the provided

- U.S. State-Issued Driver’s License
- U.S. State-Issued Identification Card
- U.S. Permanent Resident Card

Make sure the document is current and in good condition for a successful verification.
Yes. Information is securely stored. For example, you may have already created a CLEAR account to validate your identity for another purpose. This account is linked to your mobile phone number and your email. If you use a different number or email address CLEAR could have difficulty resolving your account. Once created the Clear account can be reused to validate your identity with other service providers.
Selfii only uses or shares your information to remotely verify the identity of the record requestor, for auditing purposes or other required reasons by law. You can find out more about when Selfii may share you information here: https://selfii.com/privacy-policy#whenSelfiiSharesYourInformation.
Remote identity verification requires that the patient possess a government issued photo ID. Selfii does not remotely support patients who do not possess a valid government issued photo ID. If you do not have a valid government issued photo ID accepted by Selfii, you may request your records directly from your medical provider (i.e., doctor or hospital).
Due to the rigorous ID process, it is possible there was a mismatch between some corroborating information. If you think this is in error, you should contact CLEAR at . At Selfii we are working on providing alternative ID verification models to enable users to retrieve their data. Patient Account Types
Yes, you will need a Selfii account to access their data via Selfii. You can start the Selfii account creation here: https://app.selfii.com/. A mobile device is required for a Selfii account.
Yes; you may delete your data from Selfii or deactivate my Selfii account at any time. Refer to the Selfii privacy policy (https://selfii.com/privacy-policy) to learn more.
Yes. Several approved vendor’s PHAs are connected to the Cures Gateway, and you can direct your records collected via the Cures Gateway to go into your PHA account on one of those platforms. Otis Health (https://otishealth.net) and Fasten Connect (https://www.fastenhealth.com) are PHAs that are also currently connected to the Cures Gateway. PHAs are evaluated and integrated with the Cures Gateway on an ongoing basis.
Data Availability
Selfii gathers your verified demographics from CLEAR. Your demographics, including your mobile number, will be used to build a query that is sent to all the Cures Gateway-connected health networks. Your data will be automatically retrieved if your personal records are matched with your verified identity. While the search and retrieval process usually only takes a matter of minutes, the processing of the data may take longer. In most cases, the original documents will appear in a timely manner. However, some elements of your patient medical records (such as the patient summary) may take up to 30 minutes to render. If documents appear, but your data does not appear for you to access, contact .
Patient data that is available from participating HIEs and other data sources that have connected through Cures Gateway will be accessible through Selfii to you. There is no guarantee that an HIE or other data source will be able to provide the entirety of your medical records. Selfii is currently connected to more than 10 HIEs and can also access data over national networks. If you believe your local HIE should have access to your data, but you did not receive the personal health information you were expecting, contact .
Your data will likely be a consolidation of your medical records into a single document known as a Consolidated Care Document (CCD). This document can include encounters, problems, allergies, lab results, vitals and possibly some reports. This data may be from one or more providers. Some HIEs may also provide encounter-specific documents to the PHA. Data collected by Selfii will be parsed and organized into a Selfii patient timeline. In addition, Selfii will produce and provide artificial intelligence (AI) generated summary.
Yes, you can download the original documents that were retrieved. Selfii is always seeking to improve; Selfii is planning additional types of data to download from the PHA to be available in the future.
You can download this information into a PDF document as well as a C-CDA (Consolidated Clinical Document Architecture), which will be downloadable in a raw XML format. In some cases, data that originated as text or an image may be available for download.
Contact your healthcare provider to ask about the information you were expecting to see but is not showing up through the PHA.
No; the Cures Gateway only passes data from health networks to your PHA. Your data is not stored in Cures Gateway. Your data will be stored by Selfii if you choose Selfii as your PHA. The C-CDA document will contain the source of the document.
Providers will be listed in the CCD documents (PDF). The source of the medical record collected by the PHA will be displayed in the timeline provided by Selfii.
