LANES’ collaboration with Northeast Valley Health Corporation (NEVHC) was highlighted in recent research by the California Health Care Foundation (CHCF) as a powerful use case for how health information exchange (HIE) can improve care and reduce costs. By integrating real-time admission, discharge, and transfer (ADT) alerts into NEVHC’s care coordination workflows, the team achieved dramatic results: an 85% drop in ED visits and a 68% drop in hospitalizations for patients with diabetes, and an 82% reduction in ED visits for adults with asthma. This partnership demonstrates the value of embedding timely data into primary care to improve outcomes for high-needs patients.
