To improve care for patients with chronic conditions, Northeast Valley Health Corporation (NEVHC)—a Federally Qualified Health Center serving medically underserved residents across Los Angeles County—partnered with LANES. This strategic six-month collaboration focused on enhancing care for individuals with diabetes and asthma, aiming to reduce unnecessary emergency department (ED) visits and hospital admissions.

LANES played a critical role in embedding real-time patient data directly into NEVHC’s clinical workflows. By doing so, care teams across NEVHC’s 17 locations gained timely insight into their patients’ ED visits and hospitalizations, allowing for rapid follow-up, proactive care coordination, and better chronic disease management. Through this integration and the use of targeted alerts and collaborative triage systems, providers were able to intervene earlier and more effectively.

The results were significant: NEVHC achieved an 85% reduction in ED visits and a 68% reduction in hospitalizations among diabetic patients. For adults with asthma, ED visits dropped by 82%, with stable hospitalization rates. The success of the project earned NEVHC and LANES national recognition—named one of Healthcare Innovation’s 2025 Innovator Award winners.
