LANES And Emanate Health Join Forces For Data Exchange

Partnership to benefit one million residents of California’s San Gabriel Valley

LOS ANGELES – Los Angeles Network for Enhanced Services (LANES), the leading health information exchange for Los Angeles County, has announced it will be working with Emanate Health, the largest nonprofit health care system in the San Gabriel Valley of Southern California. Emanate, formerly known as Citrus Valley Health Partners, encompasses 16 locations, including hospitals, clinics and physician offices.

With this new partnership, Emanate will be able to safely exchange patient medical records with any organization on the LANES system, which will help improve care coordination and lead to better health outcomes.

“Together, we will have the opportunity to turn health data into actionable information for providers,” said Ali Modaressi, LANES Executive Director. “ We aim to improve care quality in the San Gabriel Valley.”

Health care organizations are often siloed and face challenges such as faxing medical records and viewing limited or incomplete patient records. LANES technology is changing that by securely exchanging data between participants; as a result, providers will have instantaneously access to patient medical records.

LANES will help Emanate improve care coordination, reduce duplicative treatments and avoid costly mistakes.
