As we close out our seventh year supporting participants this holiday season, we’re grateful for continued connection, innovation, and transformation in the communities in which we live and work. And we’re celebrating just a few of the ways our participants led the state, demonstrating the real power of data sharing and its impact across healthcare and social services.
- Northeast Valley Health Corporation (NEVHC), a major FQHC with 17 locations, reduced unnecessary ED visits and hospitalizations for adults with diabetes and asthma through enhanced clinical management with LANES. The work was so groundbreaking that it won an Innovation Award from Healthcare Innovation Magazine and was included in a California Health Care Foundation report on data sharing’s impact on healthcare efficiency and affordability.
- Alta-Prospect Health System, a network of six Los Angeles County hospitals, and Partners In Care Foundation (Partners), a social service referral partner of Alta-Prospect, transformed their patient referral process with LANES in an effort to reduce patient recidivism and acute care returns among Medi-Cal beneficiaries with complex conditions. Through data-driven care coordination, there was a 68% reduction in unnecessary inpatient visits among Enhanced Care Management (ECM)-eligible or enrolled patients in one year.
- The Women, Infants, and Children (WIC) program, which delivers critical nutritional support to pregnant women, new mothers, and young children, worked with LANES to improve access to WIC’s essential services. LANES collaborated with the California WIC Association (CWA) on a pilot program to streamline referrals and care coordination, with access to health data required for WIC enrollment and ongoing participation. By automating the referral process, more new eligible individuals were successfully enrolled in WIC, achieving its goal. The pilot also now serves as a model for other WIC programs.
- L.A. Care Health Plan became the first managed care plan in California to implement FHIR®-based data exchange under the state’s Data Exchange Framework (DxF). The partnership with LANES enables secure, real-time sharing of clinical, claims, and social services data, enhancing care coordination, closing gaps in medical history, and improving outcomes for underserved communities.
- Orange County selected LANES for health information exchange services for the OC Health Care Agency (HCA). The partnership will enable secure data exchange across the HCA’s clinical, behavioral, and public health systems in support of whole person care and in alignment with the federal Trusted Exchange Framework and Common Agreement™ (TEFCA™), California Data Exchange Framework (DxF), and Behavioral Health Quality Improvement Program (BHQIP).
We remain thankful for our participants’ trust and collaboration in creating these innovative programs, and so many others, which are transforming healthcare and social services. As we look ahead to 2026, we look forward to building upon the successes and growth earned in 2025.
